adminKeymasterIt appears your environment did not allow the plugin to create a temp folder in the root of your WP installation. Try creating a /temp folder in the WP root folder, and reinstall if the error does not disappear automatically.

adminKeymasterWe have made the following changes to the field names and values of the “Social History:” section of the PDF Form, and updated your field map layout with the new PDF form.
– ALL Radio Buttons and Checkboxes work in the Social History: section
. Radio Buttons, changed name of each PDF field to be the same, and changed the values to match the respective Formidable Form values.
. Checkboxes, changed the name of each PDF field to be unique, and changed the values to match the respective Formidable Form values.
. Mapped each Formidable Radio Button to the (1) PDF field name.

adminKeymasterAs a work-around until you get the HTML emails working properly you could use Formidable’s “Default Msg” shortcode rather then the default HTML shortcode. Formidable provides information about that here.
To fix the HTML issue, you may need to request help from Formidable’s outstanding support team as our plugin has nothing to do with the email notification action other than the PDF attachment(s).

adminKeymasterThere is a setting in the Formidable Forms Email Action called “Send Emails in Plain Text”
Forms → Form Settings → Email Notifications → Options → Send Emails in Plain TextWe remain at your service.
adminKeymasterRegarding the email attachment: Open the Field Map Layout and check the option “Attach file to Email notifications” AND also check the email action item(s) you want to include the PDF as an attachment.
NOTE: All other Field Map Layouts for the same form, even tests or unfinished layouts, will be sent and/or may cause the attachment to fail if not setup correctly. Go through all unused, test, or unfinished layouts and DESELECT the option “Attach file to Email notifications” to be sure you are only sending the intended attachment(s).
Regarding the html looking email message: This is a question for the great support team at FormidablePro.com. If you do not have the “Send Emails in Plain Text” option selected in the email action, then I’d start here: Why is there HTML showing in my email?
adminKeymasterIssue resolved. We didn’t realize you were using PDFtk locally, which is not compatible with merging signatures or images. To fix, uncheck the “Enable local PDFTK” option under the SETTINGS tab.

adminKeymasterOur apologies for the delayed response. Seems to be a glitch since the last Formidable update, please select “1 SITE – 40 FORMS” then select “1 SITE – 10 FORMS”, it should work then, let us know if it does not.
