We’ve run into the same issue, however, fonts are a function/limitation of Adobe form fields, not our plugin. If the Gabriola font is not embedded you may try embedding the font into the document. If that does not work, the only option is to change the font or set the font size.

Yes, you’ll need a PDF form maker, Adobe or other. PDFEscape.com offers a free online tool which will allow you to test drive the plugin.
Approx. 700 fields are the limit, depending on how complex (server calls) the felds are.
Yes, in fact we have a ready-to-use template that includes the i9 and w4, we have also customized it to include state specific forms, probation and other company specific forms for various clients. There is one formidable form which collects all the employee’s info, and two views, one for employees to edit drafts and another for admins to print/view/download all employee forms showing needed/completed status.
We sell the i9/w4 template for $45 which includes one additional form, if needed. Or, we are happy to provide support while you create your own.

From the WP Dashboard → Formidable PRO2PDF → Under The Hood tab → Screenshot it!
Add a /tmp and /temp folder to the WP root folder and let us know if this solves your issue.

It appears your environment did not allow the plugin to create a temp folder in the root of your WP installation. Try creating a /temp folder in the WP root folder, and reinstall if the error does not disappear automatically.

We have made the following changes to the field names and values of the “Social History:” section of the PDF Form, and updated your field map layout with the new PDF form.
– ALL Radio Buttons and Checkboxes work in the Social History: section
. Radio Buttons, changed name of each PDF field to be the same, and changed the values to match the respective Formidable Form values.
. Checkboxes, changed the name of each PDF field to be unique, and changed the values to match the respective Formidable Form values.
. Mapped each Formidable Radio Button to the (1) PDF field name.
. Mapped the Formidable Checkbox to the EACH of the PDF field names.Sure, we are willing to login to your installation or, you can export the template and attach it to this form.

As a work-around until you get the HTML emails working properly you could use Formidable’s “Default Msg” shortcode rather then the default HTML shortcode. Formidable provides information about that here.
To fix the HTML issue, you may need to request help from Formidable’s outstanding support team as our plugin has nothing to do with the email notification action other than the PDF attachment(s).
