There is a checkbox example in the link above, you can also take a look here for more examples of checkbox mapping.
Radio Button Values are CASE SENSITIVE! Edit your PDF forms to make the Values match exactly to the Formidable field Values.

Using your DEMO form and DEMO (Copy) layout, please take a look at this layout: http://new.eacougar.com/wp-admin/admin-ajax.php?action=wpfx_generate&form=jybtj6&layout=25&dataset=61&format=pdf&inline=1For Radio Buttons in the Pre Registration form & dynamic PDF, you have everything setup correctly, except the Field Map Format. You must select “Show label for checkbox/select/radio” from the Format Dropdown List.

We’ve added a formatting option for Formidable Date Fields used inside Formidable Repeatable Fields – based on your request.
Here’s how our Repeatable Field Demo looks now:

Name the PDF Checkbox fields the same, and change each “Value” to match a value from the Formidable Checkboxes.
EXAMPLE:
Three PDF Checkbox fields all have the same name “Highest Ed.” with one having a value of “Grammar”, another with “High” and the last checkbox has the value of “College”.One FORMIDABLE Checkboxes Field named “Highest Ed.” with three options, “Grammar”, “High” and “College”. (But you should use a Formidable Radio Button for this as you cannot stop multiple selections with Formidable Checkboxes)
FIELD MAP LAYOUT: Map the one Formidable Checkboxes field to the one PDF Checkbox field.

Oops! I apologize, I forgot to mention that for “Multi-Select Checkboxes” you must name each of the PDF Checkboxes differently.
So for your PDF form, change “check up to five” to different names for each checkbox, then map the one Formidable Checkbox field to each of the PDF Checkbox fields, the plugin will then do the rest.

I logged into your demo server, however, I don’t see the plugin.
Try this: DUPLICATE the layout by clicking the “Duplicate this Field Map” button located at the bottom-right of the Field Map Designer screen after selecting the layout from the “Field Map to use:” dropdown.
Then try selecting the “Email action box” and let us know if it saves with the layout. If it does not, also let us know how many field mapping the layout has. You can check the new BACKUP tab for the number of field mappings.
To clarify, the “Email action box(es)” will show up below the “Attach file to Email notifications” setting when set to “YES”.
In addition to “Attach file to Email notifications” set to “YES”, you must also check the Email action box for the action you want the PDF attached to.
There are no known issues with Hostgator, or any other hosting provider with regard to attaching pdf’s to email actions. However, that said we have not specifically tested the function on Hostgator. (NOTE: Intranet installations require the Unlimited Intranet version of the plugin)
Let us know if the Email action box is also checked, and if you’re still not receiving the email attachment.
Through email we discovered the issue is caused by your server being behind a firewall and only accessible through VPN.
As PDFs are created using our web service. In order to create the PDFs on your own server, you would need to install PDFtk. Or you’ll need to punch a hole in the firewall for the plugin to access our server’s web service.

As of today’s update, the number field maps per layout has increased to a maximum of 350 per layout. Adding a 2nd layout will allow for up to 700 field mappings to create dynamic pdf files.
FYI, the 2nd dataset option can also be the same form as the 1st dataset option, so you can split the same form into two separate layouts for a maximum of 700 field mappings

Also, confirm if your server is on an intranet or the internet.
If possible, send us a screenshot of the entire Field Map Designer screen with the Formidable Form, a Dataset and the Layout selected to support (at) formidablepro2pdf DOT com? Be sure to include at least one row of the field map area of the plugin screen.
Also, let us know if you see the uploaded pdf file(s) in the upload folder… /public_html/wp-content/uploads/fpropdf-forms
Also – we released a major update today, please be sure to update your plugin to the latest version .
Your shortcut looks like it is formatted okay. Are you able to Export from the Form Layout Designer (main plugin page) where form=”1mmrl3″ AND layout=”18″?
If yes, please double check your shortcodes quote marks, if you cut/pasted here, the right side quote are not standard quote marks.
If no, please double check your PDF field name, it should only consist of letters, numbers and dashes. (no special characters)
If all the above is okay too, we may need to check under the hood. If possible, please email temporary login info to support (at) formidablepro2pdf DOT com and we’ll have a look-see and then fix or advise otherwise.

Adobe Acrobat is not required for any installation of the plugin, you may use any program which creates standard PDF Forms using text fields, checkboxes and/or radio buttons.
However, using the plugin on an offline intranet requires PDFTK be installed on your server and PHP shell commands must also be enabled. Additionally, some of the plugin’s “convenience functions” will not be available, such as the select-box of PDF fields on the Field Map Designer.
If you are able to provide a temporary login please send to support AT formidablepro2pdf DOT com, if not please provide a screen shot of the entire Layout screen with the correct form, dataset and layout selected.

– No problem!To clean up the Field Map Layouts, select the layout you want to delete, scroll to the bottom of the Field Map Designer page and click the DELETE ENTIRE FIELD MAP button.
Shortcodes do not work within email messages/actions, emails are sent when “Attach file to Email notifications” is set to YES – AND – the “Email Action” box is checked.
Does the “Export to PDF” button work from the Designer for the layout you want to send by email? Are you working online or an offline Intranet?
