PDF Checkbox fIeld “PROPERTY”, there is a setting for “Export Value”. Which PDF form creator are you using?
In Adobe Acrobat…
- Right click checkbox
- Select Options Tab
- Change “Export Value” to match the Formidable checkbox option.
In PDFEscape.com…
- Right click checkbox
- Select “Unlock Form Field”
- Right click checkbox again
- Change “Export Value” to match the Formidable checkbox option.
Save and upload to PRO2PDF to map and merge your checkbox and other data.

Here’s the short version….
For Checkboxes…
- Name each PDF Checkbox = Unique Name
- Each PDF Checkbox Export Value = A (exact) value from your Formidable Checkbox field (copy/paste for 100% accuracy) (*No Special Characters*)
- Map Each PDF Checkbox field name to the one (1) Formidable Checkbox field name (If you have 5 checkbox options, you will have 5 field mappings to one Formidable checkbox field)

For Radio Buttons…- Name each PDF Radio Button = Same (exact) Name (copy/paste for 100% accuracy)
- Each PDF Radio Button Export Value = One (1) Value from the Formidable Radio Button field (copy/paste for 100% accuracy)
- Map the one (1) PDF Radio Button field name to the one (1) Formidable Radio Button field (If you have 5 PDF radio buttons with the same name, you will only have one (1) field mapping to the Formidable radio button field)

“Forms” are created using Formidable Forms, which offers both a free version and premium version.
This plugin is an add-on to Formidable Forms which merges form data to pre-made PDF forms. The free version allows one Formidable form to merge data to any number of PDF forms. While the premium version allows additional Formidable Forms to merge data to PDF Forms and offers additional features, such as attach merged PDF’s to email actions, signatures, repeating sections, embedded forms, and more.
The premium version of this plugin does have an annual cost, however, we are able to offer extended years (up to 5).
Not currently scheduled, however, should be 3-4 days.

Apparently your hosting provider disabled the /tmp folder, we have corrected the issue on your server and all appears to be working as expected.

Yes, correct. Are you able to send the error to your hosting provider? It appears you are prevented from creating a tmp folder, but is an error we have not seen in the past.

It appears to be a bug, but does not affect the plugin. We will correct upon the next update unless you are having any errors or the plugin not working as expected.

The plugin does not limit the number of pages on the PDF, you may upload pdf’s of any size to both the free and premium versions.

That’s odd…. Are you having any errors or is the plugin still working as expected? You may try uninstalling and reinstalling the plugin to remove the notification.

Open all unused, test, and failed Field Map Layout and change”Attach file to Email notifications” to “NO” and click “Save Field Map”.
Or, it’s better to remove all unused, test, and failed “Field Map Layouts” that are associated with the Formidable Form.
To remove all unused, test, and failed “Field Map Layouts”;
1. Select the “Select the form to export data from:”
2. Select the “Field Map to use:”
3. Scroll to the bottom of the field mappings and click “DELETE ENTIRE FIELD MAP”, and confirm the action.
4. Repeat until all unused, test, and failed “Field Map Layouts” are deleted.Then test and let us know the outcome while we remain at your service.
