Regarding the PDF File Name, by default, the name will be the same name as the field map layout. Optionally, there is a setting called “PDF file name in e-mails:” on the field map layout, or, if you have the paid version you can use filename shortcode parameter to dynamically change the filename when PDFs are downloaded by your users. NOTE: This feature does not work for the filename of PDF attachments.

It sounds like there is another field map layout that is trying to attach a PDF and is failing. Check to be sure all other field map layouts for the same form are deleted. If they are, try recreating the form’s email action and delete the old email action.

Your Formidable checkbox values have been updated (by adding $2) since creating the PDF form. The PDF form has the old values.
To resolve the issue, edit the PDF Form Checkbox Values to match the Formidable Checkbox Values, and upload the new PDF to the plugin. All will work as expected without any further updating.

Yes, you must first create, upload, and map a fillable PDF form to the plugin. The plugin does not ‘create’ PDFs, it just merges form field data to PDF form fields.

Actually, in your case it sounds like the shortcode is formatted incorrectly, please copy/paste the shortcode from your On Submit Message box to the reply message box below for further evaluation and review.

Yes, and yes. Images uploaded through a Formidable File Upload Field may be mapped. (NOTE: Only one image per upload field) You are able to map signatures using Formidable’s Signature plugin.

Make sure the PDF Radio Buttons have the same “RADIO BUTTON CHOICE” as the Formidable field options. If using Acrobat, the “RADIO BUTTON CHOICE” is located on the Radio Button Property Screen under the OPTIONS tab.
NOTE: Only use Radio Buttons for single option field choice, use Checkboxes for multi-option field choices. PDF Checkboxes have an “EXPORT VALUE” located on the Checkbox Property Screen under the OPTIONS tab.
NOTE: The RADIO BUTTON CHOICE or EXPORT VALUE must match the PDF Field Option exactly, we suggest using copy/paste to be sure they 100% identical.

Sounds like the PDF was created in LiveCycle rather than Acrobat, or there are PDF signature fields in the document. If so, the plugin is not compatible with LiveCycle or Adobe Signature fields, revise the PDF to fix the issue. Our plugin merely maps Formidable form field to PDF form fields, it does not create Formidable Forms or PDF Forms. Formidable Forms and PDF Forms must first be created then our plugin may be used to map the fields to provide a dynamic PDF output.
Yes, the intranet version includes all features except signatures, images and the support for Unicode language font which supports Thai, Chinese, and Eastern European.

Yes, sounds like you have unfinished, unused, abandoned, or non-working field map layout(s) for the same form that also have email attachment checked. Go through all other field map layouts for the same form and uncheck the email attachment option. Or better yet, delete all unused field map layouts by selecting the layout – scroll to the bottom of the layout and click “Delete Entire Field Map”.

Logo images may be mapped using the paid version, however, PDFs are not HTML compatible by design so we have no way to map HTML fields to PDFs.
