adminKeymasterIf the Formidable is a radio button then the related PDF fields should all have the same name but PDF fields Export Values should match each of the Formidable radio button options.
If the PDF fields are named the same as the Formidable field name or key then the plugin will automatically map the fields, if the name/key does not match, you must manually map the one Formidable field to the one PDF field name/key.

Or, you can create a new form that imports the required data from another form using Formidable’s Get Value from Entries shortcode then create the field map layout to map your PDF.

adminKeymasterThe easiest solution is for the email to have links for you to download the applications that have been selected.
In order to attach the selected applications, you’d need to create a different form for each application and have the user complete and submit them separately, which could be made to appear seamless by changing the SUBMIT button text to NEXT>, but is a lot more work to setup.We remain at your service.

Based on your request, it appears you have been able to setup field map layouts so you can see forms in the “Select the form to export data from:” dropdown, but now you are unable to see one paticular form – is that correct? What happens if you add another test form, can you see that form? If yes, try changing the name of the form or duplicate the form and let us know if you can see the duplicate.2)
To delete previous field map layouts, from the Export Tab… select the layout from the “Field Map to use:” dropdown and click the “Delete Entire Field Map” button at the bottom of the field map layout.3)
Please elaborate on your last question: “I am unable to do anything is the “format” column, I thought the form image would look similar to the on in the image but it does not – is that section a change to the plug-in?”We remain at your service.

To fix, select the blank named field map layout from the “Field Map to use:” dropdown field, then enter a name into the “Name of Field Map (will be used as default filename):” field and click the “Save Field Map” button at the bottom of the field map layout.
If this does not solve the issue, send a screenshot of the field map layout with the blank field map selected in the “Field Map to use:” dropdown field to support AT formidablepro2pdf DOT com, or complete this form to have us take a look at your installation.
