Hi Barb,
The switch form menu is a Formidable Forms dropdown, not our third-party add-on plugin. We are using auto-size for all of our form drop-down lists.
Let us know if it is our plugin you are finding the issue, as we remain at your service.
adminKeymasterDouble check the “Format” dropdown for the mapped fields in the Field Map Layout, should be set to “No Formatting”, if it is… then to further assist please provide a screenshot of the Field Map Layout screen with the Form, Dataset and Layout selected and to include the mapped fields. If we don’t see the issue there, we may need to look at the PDF form and possibly the Formidable Form.
Or complete this form to have us login and take a quick look…
adminKeymasterContact support AT formidablepro2pdf DOT com for consideration.

January 22, 2016 at 5:59 am in reply to: There was an error generating the PDF file. This form has not been activated yet #5802adminKeymasterIf you are able to provide temporary admin access to your WP installation, complete this form and we’ll check it out quickly.
If not able to provide access, please email a screenshot of the entire plugin screen with the Form, Dataset and Layout selected, as well as, a copy of the shortcode you are using to support AT formidablepro2pdf DOT com for further review.
adminKeymasterYes, but you’ll activate / deactivate from the WP plugin screen and Formidable’s Global Settings page.
We remain at your (quick) service! 😉
adminKeymasterUse the Activated Forms tab to activate/deactivate each site with the purchased licence key to change site.
adminKeymasterWe understand your need to change the PDF page output based on the form’s submit data, we too had the same need.
While the plugin will not change the structure of the PDF, and there are no intentions to add any PDF structure changing ability to the plugin, you can make the LayoutID variable and dynamic based on the form’s data, which allows you to have multiple Field Map Layouts from one Formidable Form to many PDF forms.
For your particular scenario we would:
. Create a master PDF Form (Form-with-images.pdf) which includes all possible fields.
. Map the master PDF to the Formidable Form (Form with images).
. Edit the master PDF Form to remove image page(s) and SaveAs Form-without-images.pdf
. Create a Duplicate of the “Form-with-images” field map layout by selecting the layout, then click Duplicate at the bottom of the map.
. Upload the new “Form-without-images.pdf” to the Field Map Designer and save it to the new layout.
. Delete the unneeded fields from the new layout. Now, you’ll need to make the LayoutID dynamic…
Use the Formidable Shortcode Conditionals such as IF STATEMENTS directly in your shortcode, or to decide which shortcode to use or not to use…Please let us know if you need more help with this issue as we remain at your service.
Here are a couple of forum discussions about ways of making the shortcode dynamic…

http://www.formidablepro2pdf.com/support/subject/the-use-of-more-than-one-pdf-in-one-form-problems-with-the-download-link/adminKeymasterThank you for using the plugin. The plugin does work with repeatable sections, and will only merge what the user inputs. Please provide more information regarding your question, “Is it possible to only show the sections added by the user on the frontend form, so the pdf only shows the added sections?”
Have you created the PDF form? If so, please email the form or screenshots to support AT formidablepro2pdf DOT com with more details about what you need and/or don’t need merged.

adminKeymasterThank you for using the plugin. Even thought the Field ID still shows, the plugin is actually using the keys once you select the “Use field keys instead of field IDs” option.
We will look at providing a batch export for the next release based on your request.

adminKeymasterThe free version does not expire! The $2.00 trial version expires after 30 days. If you have the trial version, email the registration key to support AT formidablepro2pdf DOT com and we will reply with the expiration date.
If you are using the free version, and you have all the features you need, then you are all set – for free – forever!
adminKeymasterSelect the undesired layout, then click the “Delete entire field map” button at the bottom of the field map. Then confirm the delete action.
Thank you for using the plugin.

adminKeymasterThank you for using the plugin!
If you really do like the plugin, or support, or both, we would be grateful and honored if you would provide a review on the plugin’s wordpress.org page and/or on the plugin’s site.
It’s really sad… we only have 8 on WordPress.org, where they count the most…
adminKeymasterHi, thank you for your patients and for submitting this issue on the support forum, as this is an issue we have not seen before…
The font type you used “Kozuka Mincho Pr6N R” is not happy with being used in a PDF form, I’ve changed the font to many other types and all works as expected… (other issues with this font are found on Adobe’s site…)
SOLUTION: Change the PDF form field fonts to nearly anything else…
When updating the PDF form simply edit the PDF on your computer and upload the PDF using the Field Map Layout screen, the plugin overwrites any PDF with the same name and there is no need to remap the fields*.
*Even if you change the PDF name, there is no need to remap the fields, simply select the Form and Field Map Layout from the top dropdowns and change the PDF Form dropdown to the newly named form. The plugin will recognize the field names and map to the new PDF filename.

adminKeymasterWe got the screenshot, but need we need to check the actual PDF form. Please provide the PDF.
Sounds like there may be duplicate fields sitting on top of the mapped fields, which could happen in Adobe Acrobat if you select all fields then move while Ctrl is depressed.
adminKeymasterI received the screenshot, but need to check the actual PDF form. Please provide the PDF.
adminKeymasterThis is not an issue we have seen before. Please email a copy of the PDF form to support AT formidablepro2pdf DOT com?
January 15, 2016 at 8:41 am in reply to: Checkbox issue, one check is fine, multiple shows empty #5693adminKeymasterOur apologies, we should have mentioned in the first message that if you are using Formidable’s “separate values” option and you are not using the “label” (‘display value’ as referred by Formidable) as the PDF value, then there is no need to set the Format dropdown to “Show label for checkbox” in the Field Map Layout…
Changing the format setting to “(no formatting)” in the Field Map Layout corrected the issue, and now all works as expected.
Thank you for using the plugin and for your question on the support forum, we will update the checkbox documentation to better explain the ‘label’ settings, options, and uses.

adminKeymasterThe plugin is not compatible with LiveCycle forms, it must be a PDF Form.
Solution: Use Adobe Acrobat or PDFEscape.com to recreate the PDF.
If you absolutely need the LiveCycle form to work with the plugin, we could create a customized version for you. If so desired, please complete this form to receive an offer.

If admin login is possible, please complete this form to have us take a look at the installation.
If login is not possible, please email a screenshot of the entire field map layout with the form, dataset, and layout selected as well as all the mappings for further review.

January 15, 2016 at 6:20 am in reply to: Checkbox issue, one check is fine, multiple shows empty #5687adminKeymasterIt sounds like all your PDF form’s checkboxes have the same name… To configure Formidable’s Checkbox fields for “Multi-Select” values…
. Enter a unique name for each PDF Checkbox.
. Change the “Value” of each PDF Checkbox to match one “Value” of the Formidable Checkbox. If you have setup like the above directions, please email a link to the Formidable form and a copy of the pdf for further assistance to support AT formidablepro2pdf DOT com.
adminKeymasterNo worries!!! This is a good support thread because it really does look like a read-only or disabled field. It’s a lot like a Windows Username & Password login box when there is no password set, most people will never try “no password” and be locked out forever…
Thanks for using the plugin, do not spend a lot of time trying to figure something out if something doesn’t seem to be working correctly, check this forum and if you don’t see it, then don’t hesitate to ask us, as we are truly at your service!
adminKeymaster🙂
It was not marked as read-only, the grayed out text is the ‘default filename’ you can just type over it. 😉
Give it a try and let us know if you experience any issues while we remain at your service.
adminKeymasterYes, please provide credentials by completing this form. We like to see exactly what is causing the plugin to disable the field. Thank you.
adminKeymasterThank you for the screenshots… Found it!
Remove the “.” (period) from the PDF Form filename, then reupload and change the PDF Form in the Field Map Layout.
Let us know if that fixes the issue as we remain at your service.
