Great question, thank you for trying the PRO2PDF plugin.
Without further programming, you could dynamically change the “Layout ID” in the shortcode, allowing you to map multiple PDF forms with only the required data fields in variations of your main form.
For quick execution I would;
. Upload all PDF form variations
. Create a Layout and Map the “full” PDF form – with all required fields
. Create a copy of the layout (button at bottom of the Field Map Designer) for each variation of the “full” PDF form
. Delete the unwanted field mappings from the new layouts
. Create a field in the Formidable Form for the “Layout ID” which dynamically changes based on the conditional logic.
Creating PDF’s in a Saved Draft is accomplished by placing the PRO2PDF shortcode in the Messages|Saved Draft section of the Formidable Form Settings|General screen.

The issue is likely missing software on your server. PDF files use UTF-16, not UTF-8, so we use the PHP MBstring module to re-encode text, which is available on most servers: http://php.net/manual/en/mbstring.installation.php
There’s a fallback when the module is not installed: http://php.net/manual/en/book.iconv.php
If you can install one or the other the issue should go away. If one or both are already installed on your server, we may need FTP access to check the issue. In that case if you can provide credentials please send to support(at)formidablepro2pdf.com.

Yes, check boxes can be mapped the same as radio buttons, by setting the “EXPORT VALUE” of the PDF form field to the value of the individual Formidable form check box value.
While testing today we found an error when multiple check boxes are selected for the same Formidable Form field. A temporary work around is to add and map a separate check box field for each PDF form field.
We are looking into a fix and I’ll message you as soon as we have an update.

We’re currently working on a “Add a second dataset” demo and video, however in the meantime let me know if I can login to your test server to assist you in making the forms work with the plugin. If it’s possible then email temporary login credentials to support(at)formidablepro2pdf.com.

For a second dataset your shortcode must have two layouts, in your example you only have layout=18, but the second dataset must have it’s own layout.
1. Be sure both forms are active in the PRO2PDF Plugin’s “Activated Forms” tab (ASSESSMENT REPORT and PERSON FORM)
2. Create a second layout for the student’s name (Use the same PDF Form, but must map student name from PERSON FORM to Student Name in PDF FORM)
3. There must be at least one dataset in each Formidable Form for the mapping to work.
I’ve created a demo for you to view, I will email the login credentials to you.
Use the “Add a second dataset” feature to access the Students Name dataset from the PERSON Form, and change the shortcode’s dataset2 setting to dataset2=[dropdownID].
There was a conflict in CSS styles between our plugin and Fusion Builder. It’s solved, so the layout builder works fine now.
There’s a new update of the plugin available on WordPress.org. You can update it on other sites if you go to WordPress Dashboard → Updates.

I viewed the video but did not see if there was a dataset for the Reservasi Form.
Please select the tes_2 layout, and the Reservasi Form on the Formidable PRO2PDF page and make sure you see the two entries listed, and select one of them.
“Do I need to type PDF Form Field Name based on the name of my field name in FP form?”
No – the field names from the Formidable Form and PDF Form will automatically appear in the dropdown lists as long as the Formidable Form has at least one dataset.
You must submit data to the formidable form at least once. From your screenshot it appears there are no data entries in the Formidable Form. There must be at least one dataset before the plugin will allow you to map the fields.
Please submit data to your Formidable Form, then return to the plugin page and select your layout to map the fields. Let us know if that solves the issue.
Correct! You’ll then see a link to download the filled-in PDF Form – with the data that was just entered into the Formidable Form – followed by “Your responses were successfully submitted. Thank you!”.

We have tested the plugin with the Avada theme and found no errors, however we do not have the Fusion Builder to test. Are you able to provide a zip file of Fusion Builder for testing? If so, please send to support(at)formidablepro2pdf.com
According to the example in your question, you are attempting to place the shortcode in the Formidable Form’s CUSTOMIZE HTML area, which is not the correct location.
Instead, place your shortcode in the Formidable Form’s SETTINGS >> GENERAL (tab) >> ON SUBMIT MESSAGE (text box) and please let us know if you require further assistance.
We released an update this morning that adds new date field formats for both current date and user-submitted date fields for (mm/dd/yyyy and dd/mm/yyyy and yyyy/mm/dd)
Multi-line text fields seem to appear correctly on our server however we added a tweak that should solve the problem, and it should work as expected on all servers. Please let us know if it works now on your server.
We will add the date format option and will look at the paragraph marks within multi-line text fields over the next day or two and I will notify you as soon as the update is ready.

We will soon revise the multisite rates, but for now you can note the number of sites you want to make active in the comment section of your order, which will be processed manually. For now, you can request any combination of sites to forms, for example, with the 1 Site – 10 Forms option you could request;
10 Sites – 1 Form or
6 Sites – 5 Forms or
2 Sites – 9 Forms
And the same would be for the 1 Site – 40 Forms.
You may also request additional forms at $1 each.
This service offering is intended to allow developers to include the service in many projects. We realize this is confusing and are working to develop a solution.
Sounds like you found the right plugin!
Here’s an overview of what steps you can take;
- Using Formidable Form’s plugin – create one form with fields for all the required information to complete both tax forms. (Note: You must submit at least dataset record to use the PRO2PDF plugin)
- Using http://www.PDFEscape.com – add PDF Form Fields to blank W-9 and W-2 forms.
- Using Formidable PRO2PDF – upload the PDF Forms and create two Field Maps – one for each PDF Form.
- Place both PRO2PDF shortcodes in the Formidable Form’s On-Submit message, but change the dataset=’xx’ to dataset=[id] so the user can download, print or email their forms.
Let us know which steps you need more detailed instructions.
