Duplicating the Field Map Layout is the correct way to accomplish desired result. After duplicating the layout and uploading the new PDF form – with the same PDF form field names – your layout should stay mapped and you should not lose mappings except the “missing” pdf fields.
The template function was not designed to create duplicates as it will override previously saved layouts with the same name.
Layout ID’s can be found in the resulting shortcode after selecting the layout from the Field Map to use: dropdown located in the plugin’s export tab.
We are happy to login and check the “duplicate” layout function if you complete this form.

Open the plugin’s field map layout for the pre-registration form and see if the “Email Notification” is checked, if not, check it and test. If it is checked, then un-check it – save – reopen and recheck it – save – and test.

Upon log on we were able to see the plugin screen and submitted demo data to the demo form which merged the PDF as expected.

You’ll need the 1-SITE | 40-FORMS subscription, and we’ll change it to 2-SITES | 39-FORMS for you. Just let us know once you have your order ID and we’ll make the change manually.
A multi-site install requires 1-SITE per site on the server. But! You can mix subscriptions;
EXAMPLE: If you purchase (1 SITE | 10 FORMS), you can mix the subscriptions like this; (3-SITES | 8-FORMS) or (10-SITES | 1-FORM)

Not sure, but usually a blank screen is a CSS issue with another plugin or the theme. Revolution Slider recently caused a similar issue that was fixed by updating the slider plugin.
If possible, please complete this form to have us take a look at the installation, or try disabling other plugins to see if you can find the cause. If it is not a plugin, then try changing the theme, and let us know what you find so that we may try to duplicate and fix the issue.

Have you entered data into the Formidable Form? If not, you must submit data at least once before you can create a field map layout.

Try using the BACKUPS tab to restore the most previous working layout, then determine what change caused the error and let us know the results as we remain at your service.
After one year, if you do not renew your subscription the plugin reverts to the free version (1-Site | 1-Form )
. Offline requires you to install PDFtk on your WP server, but you lose the Signature and Image functionality, however, the plugin does not stop working after the subscription period has ended only the Support and Updates will end.

After one year the Pro “Internet” Version will expire if not renewed, however, the free version will still work (1-Site | 1-Form). Subscriptions can be renewed at any time, or even purchased for up to 5 years.
Alternatively, the Intranet Version will continue to work after one year, only the support and updates will expire. The Intranet Version requires PDFtk to be installed on your WP server.

2. If you have a form condition that changes the form from Draft to Final, such as a radio button, you can map the button’s value to map a PDF Form text field with “watermark like” font settings and is set behind other fields in the PDF form. Using Formidable’s “Use different value”, with “DRAFT” as one value, and a blank as the second value.
