Yes, if we understand correctly, where you have numbers is where a PDF form field will go, could be a checkbox or a text box, at first glance we would suggest a checkbox depending on how you want to input the data into the web form.

There are no known outstanding issues with checkbox fields. Checkbox fields DO NOT work the same as radio fields, as radio fields are named the same and checkbox fields are named differently. Both require that the PDF checkbox field “Export Value” be the same as the Formidable checkbox field value.

Yes, the Formidable Forms plugin provides an “Entries” page for each form which allows for exporting, custom views that can be created with links to merged PDFs, as well as shortcodes to display form data in a table format.

The plugin/shortcode creates one PDF per dataset or ‘record submitted’. There are currently no options to merge all records into a single PDF, however, a list of entries with a link to PDF’s for each is possible using Formidable’s Views.

We are fixing the issue, however, a work-around is immediately available by changing the Webform Data Field for each “Image URL” field to a file upload field, then set the “Image Alignment” option – SAVE – then change the Webform Data Field back to the “Image URL” field, and save.

The Address format option works like the repeatable field format for Formidable’s new Address field group type.

This is not related to Paragraph Text Fields. It appears the issue is the way you’ve mapped multiple Formidable text fields to one PDF text field, so, if the last merged field is blank, the PDF text field will be blank.
To resolve the issue, add PDF text fields so that you can map one Formidable text field to one PDF text field. The PDF text fields will be transparent so only the one ‘visible’ Formidable field will be visible in the PDF.

This was something we had not seen before, your unique WP environment required a change to the source code by adding a is_readable check to the condition for selecting a tmp folder, rather than is_writable.

We’ve seen this before;
1) Confirm you DO NOT have any PDF Signature Fields or LiveCycle PDF Fields in your PDF Form.
2) Confirm you have at least one data entry entered into the Formidable Form.If the above is confirmed, and you still do not see any fields, please send us a screenshot of the plugin’s entire “Export” tab page with the Formidable Form and Layout Selected, or complete this form for further assistance.
