Yes, you must upload a PDF Form, then map the PDF form fields to the Formidable Form fields, then each merged PDF document may be instantly downloaded using the shortcode or emailed using Formidable’s Email Actions.
The merging the data to the PDF forms and the instant download functionality may be tested with the free version, however, the email functionality requires the paid version.

The email error message may not be accurate, it appears the PDF merge is failing, and not an activation issue.
Try unchecking the “Send Email Attachment” option on all unused, unfinished or test Field Map Layouts or better yet, delete them. Then test the merge from the plugin’s admin screen, if it works then test the email attachment.

Regarding PDF Downloads: The “pdf” can always be recreated on your WP server with a shortcode as long as the form, data entry, and field map layout all exist. If you need to automatically store each PDF file on your server, we do have a $10 “Save-to-File” add-on plugin that will save each PDF that is generated to a pre-designated folder on the WP server. The code may be further customized to provide additional functionality with each PDF. If needed, please complete this form and we’ll send the add-on to you.
Regarding Emails: The shortcode does not have an email attachment parameter, rather, the option to attach PDFs to emails is selected in each Field Map Layout. If the “Email Notification” is checked, the merged PDF will be attached to every email that is sent. The “dynamic example” uses the conditional logic built into Formidable’s email actions to send the email, or not. As an example, you could create two identical email notifications but only check one of them in the Field Map Layout, then use Formidable’s conditional logic to choose which email notification to send, with or without attachment…

To provided examples of checkbox and radio button fields, we made the following corrections to the PDF Form’s “Current Address” and “Check all that apply” fields…
“Current Address” (Radio Button field)
– Named all radio buttons the same, “Current Address”
– Changed the “Export Values” to match the Formidable options (they were all set to ‘Yes’)“Check all that apply” (Checkboxes field)
– Changed the “Export Values” to match the Formidable options (they were all set to ‘Yes’)… and, we made the following changes to the Formidable Form “Current Address” field…
– Changed the field type to Radio Button (for single selection functionality)…and, we made the following corrections to the Field Map Layout, and renamed the field map to “ApplicationMerge”…
– Mapped [156] Type of Residents to the one “Current Address” PDF radio button field
– Mapped [196] Check all that apply 4 times to all 4 related and uniquely named PDF Checkboxes
– Deleted all additional field map layouts for the “Application to Rent” form (duplicates and abandoned field map layouts may cause email attachment issues)
– Checked the “Limit Field Map to use Dropdown” option under Settings (best setting to know which field maps belong to which Formidable forms)Revised PDF is attached and has been uploaded to the site.

Field values must be exact, they are case sensitive and spaces matter. We strongly suggest using copy/paste to ensure that data in mapped checkbox and radio button fields are identical.
Additionally, the plugin does not like some special characters such as % / \ | ] [ and others, if your Formidable field value must contain special characters then you should use the “Use Separate Values” Formidable field option to set the value to something like 1, 2, 3 or A, B, C.

The ‘other’ option in radio and checkbox fields can be used correctly if the result is merged into a PDF text field. Otherwise, use a Formidable Text Field and show/hide based on when the ‘other’ option is selected.
