Our PRO2PDF plugin does not calculate, combine fields, or offer javascript functionality and we do not offer support for JQuery or Javascript.

UPDATE: We expect more information on our attempts to resolve the known issue in (1) one week.
Changing the PDF font may resolve the issue, however, there is a known bug with Slovenian, Croatian, Slovak and Czech characters (Č,č, ć, Ć, ž, Ž, š, Š) and we are currently working to resolve the issue. Full refunds are available if you find the plugin is not able to render the characters required in your document.

Our understanding is that, “UETA/ESIGN compliant” is only required when explicitly stated in the document’s electronic signing/filing instructions. However, only the lawyers really know… and they too argue about it.
We have been using Formidable’s signature plugin for all our tax forms and insurance client signatures since 2013, without any issues.
Great question! The phrase “legally acceptable in the USA” is relative to the instructions or guidelines of the document or agency.
For instance, some documents/agencies may require “UETA/ESIGN compliant”, however, for the W-9 the signature is required to be from a system that “authenticates and verifies the submission“, which a WordPress user account accomplishes.

This is not “built-in” to Formidable, however, you can achieve the desired result by redirecting the user to a Formidable View with a ‘Confirmation” form after they click submit. Then, if not confirmed, redirect them to EDIT the original form.

Yes, that’s correct. We recently added an Automatic Field Map Layout feature, so if you name the PDF fields the same as the Formidable fields, they will map automatically. All except multi-select checkboxes.
You must first create and upload your PDF form with the form fields already in the document, then you’ll be able to map the Formidable fields to the PDF fields. From the Field Map Layout screen, select the “Consent Form – 14 – 17 yrs old” form from the “Select the form to export data from:” dropdown, then one-at-a-time, select each of the test/unused layouts from the “Field Map to use:” dropdown, then un-check each of the email actions listed under the “Attach file to Email notifications” from the layout settings.

There are multiple field map layouts for the “Consent Form – 14 – 17 yrs old” form that have the email actions selected, please de-select email actions from all the test and unused layouts, as the plugin is designed to send email attachments to all layouts upon submission of that form.
Regarding the signature issue, you must select “SIGNATURE” from the format dropdown on the field map layout screen.

Oops… Sorry, I just noticed (after coffee) you were asking about SITES not FORMS… My apologies.
You just need to let us know how to setup your subscription. You can have up to 10 Sites OR Forms, or you can mix and match, such as 6 SITES | 5 FORMS.

Formidable Forms will store the data in the database, you can print (or not) the PDF at anytime.
For our w-4’s, we add the annually updated w-4 forms each year to the plugin and then make the link’s layout ID dynamic, so at anytime we can print the correct w-4 for the year they completed the online form…
We have not experienced any issues with MS servers. At this point, we would need to see the installation and may also need FTP access to find and fix the cause.

Try uninstalling and reinstalling the plugin, also try disabling other plugins and changing the theme to see if there is a conflict.
If the plugin does not work with the Demo form, and if the plugin has never worked on your server, then we have no other suggestions, we would need to review the installation.
Sounds like something may be wrong with the PDF form. Does the field map layout work with the Demo form? Please complete this form if we can login to your WP installation to take a look.
NOTE: The plugin is not compatible with LiveCycle forms.
We also tested the demo that is built-in to the plugin and there appears to be another issue which requires our senior developer to take a look at, this ticket has been forwarded to him.
The form you are trying to map to is a LiveCycle form, which is not compatible with the plugin. You’ll need to create a PDF form using Adobe Acrobat, PDFEscape.com, or any other PDF form creation program.
Please recreate the W-9 form and we’ll be happy to create a radio button example mapping for you.
