Formidable Forms will store the data in the database, you can print (or not) the PDF at anytime.

For our w-4’s, we add the annually updated w-4 forms each year to the plugin and then make the link’s layout ID dynamic, so at anytime we can print the correct w-4 for the year they completed the online form…

Try uninstalling and reinstalling the plugin, also try disabling other plugins and changing the theme to see if there is a conflict.

Sounds like something may be wrong with the PDF form. Does the field map layout work with the Demo form? Please complete this form if we can login to your WP installation to take a look.

NOTE: The plugin is not compatible with LiveCycle forms.

The form you are trying to map to is a LiveCycle form, which is not compatible with the plugin. You’ll need to create a PDF form using Adobe Acrobat, PDFEscape.com, or any other PDF form creation program.

Name your PDF Radio Buttons the same, but change the PDF Radio Button “Value” to the value of the Formidable Radio Button. Then, map the Formidable radio button to the PDF radio button.

NOTE: Formidable’s “Use Separate Value” setting is not required, it is optional.
NOTE: The PDF Radio Button “value” must be exactly the same as the Formidable field value, use Copy/Paste to be sure.

You must delete all extra, test and unneeded field map layouts or change the “Attach file to Email notifications” setting to NO.

If the Field Map Layouts are grayed out, then deselect the “Limit Field Map to use Dropdown” setting on the SETTINGS tab of the plugin and try again.

The issue is being caused by the Formidable Form. I created a test signature form and it works.

You can see from the “Entries” screen that the form is not saving the signature image because it’s missing, then if you open the “Sig test” you’ll see the signature is saved and visible.

I created a duplicate of your form for testing and it appears the signature field works if you move it to the last page of the Formidable form. So Formidable Form is failing to save the signature if the signature field is on the first page, I didn’t try any other pages except the last as we are way out of the plugin area of support… At this point you can open a ticket with Formidable, pay us to open a ticket with Formidable, or move the signature to the last page… (or test other pages…)
