A: Yes, all versions have unlimited PDF form capability. The license form counts are regarding the number of Formidable Forms that are able to map to any number of PDF forms.

The free version of the plugin requires internet access. For the Intranet Version is required for use on an intranet. You’ll need to test the free version on a web-based WP installation.

After reviewing your installation and testing the plugin we were unable to find any issues, until we took a closer look at your PDF form and discovered that all the fields were set to a font color of “white”…

May be a font issue, which font are you using in the PDF?
May be a shortcode issue, does the plugin export PDF’s correctly from the EXPORT tab after selecting form, dataset, and layout?

You MUST have PDFtk installed on the local server, and, from the plugin’s SETTINGS tab; you must check the option “Enable local PDFTK”.

The issue is arising from the “Postmark (Official)” wp_mail replacement plugin. When the plugin is deactivated the form submits without error and the PDF is attached to the email, when the plugin is active the form submit results in a 500 error.
If you must use the “Postmark (Official)” plugin a work around would be to add the PRO2PDF shortcode to the email body to allow the user to click-to-download the PDF form.

The issue was within Formidable Forms, caused by using a variable in the option “Yes – Customer & [first_name] will receive emails w/attached forms.“, we changed the conditional logic to NOT EQUAL TO “No – I prefer the extra work of downloading them.” and all works as expected.

Based on the description of your issue it sounds like a question for the great team at Formidable Forms. Once the email is sent and received the PDF should be attached as expected.
Take a look at this support thread before entering a support ticket with Formidable, it may solve your issue.

Regarding: “the upload file link would not take my file. I uploaded it to the media folder in wp and left a link to it in the submission. I forgot to add….”
The PDF form MUST be uploaded to the PRO2PDF plugin from the EXPORT tab, there are no other options, it must work. If the PDF form is too large (more than 4mb) you’ll need to shrink the form.
Regarding the 1,2,3,4 list above, what you are describing, or are you requesting support?

Try this…
1) Check for any other field map layouts using the same form and make sure they too have the same field selected as the dataset name.
…. if that doesn’t work…
2) Try deleting the mapping for the field that you want to use as the dataset name, save the field map layout, then add the field mapping back to the layout and select it as the dataset name.

Name each PDF Radio option in a group the same, but you must change each PDF “Radio Button Choice” (also known as “Export Value” for checkboxes) exactly the same as their corresponding Formidable Radio Button Option value.
