Sounds like there are one or more issues with the PDF form. Upload a copy of the PDF form that was uploaded to the plugin to this form so we may take a look at what may be causing the error message and the missing field previews.
Regarding the grayed out settings, can you also upload us a screenshot of the plugin’s “Activated Forms” tab?

. Disabling all plugins except Formidable Forms and PRO2PDF, then test for the issue.
. Change the PRO2PDF “Attach file to Email notifications” setting on the field map layout to “NO”, then test for the issue.

One form means one “Formidable form”. The plugin allows you to create an unlimited number of field map layouts for each Formidable Form. Let us know exactly where and when you are receiving an error for further support.

Yes, download the free version and change the field map layout’s Language setting to “Unicode”.
