It appears the issue is being caused by one of more of the Repeatable Section Fields in your form. We’ve imported and tested your form on our server and all appears to work as expected, although this is a very large form of required fields, and numerous variables may be different from your installation to ours. Additionally, some of the required fields are causing odd errors in Formidable, which may be related or may not.

We’ve also spent a good deal of time dissecting duplicate Field Map Layouts on your site, however, have not yet found the cause of the issue.

Could you assist us in troubleshooting by creating a simpler version of one repeating section form and map it to your form to see if the function is causing the issue?

We have not experienced the issue you have described, in the past month, on the hundreds of installations running the PRO2PDF plugin, including several of our own.

If you have the Pro version of Formidable Forms installed, go to your WordPress Plugins page and deactivate Formidable. Then select ‘Delete’ or go to your web server (via FTP, SSH, SFTP, etc.) and delete the entire formidable folder. If you are using the free version, please delete and reinstall from WordPress.org here. You will not lose any saved forms or entries when you delete Formidable. (from Formidable’s site)

After reinstalling Formidable Forms, please check for the issue again and let us know the results.

If you would also like to reinstall PRO2PDF, be sure to first export your field map layouts before deleting and reinstalling the plugin.

First,I need the signature to appear at every page in pdf….
. Add a PDF text field to your form where you “need the signature to appear at every page in pdf” and then map the PDF text fields to one or more Formidable signature fields. (PDF text fields named the same do not “duplicate” signatures, uniquely name each and mapped in the field map layout.)

Second, I need the name field to appear on 2 pages….
. Name the two PDF text fields the same on your PDF form, then you just need to map the field one time in the field map layout.

Third, I am not sure how to make repeated field show vertically….
. Not sure what you mean by vertically, however, with repeatable fields you have two format options,
1. Repeatable – Allows the manual entry of repeatable field ID’s [xxx] and html formatting.
2. Repeatable (separate fields) – Allows the manual entry of repeatable field ID’s [xxx] and html formatting for placing data in separate fields, rather than consecutive fields.

For option 2, name the PDF text fields the same but add 1,2,3,4… at the end of each field.

By design (of every application in the world) Radio Buttons are a single select option. You must use a Checkbox to select multiple options for one field.
