I would like to use this plugin and purchased the trial to play with it. I first noticed the software is looking for a dataset. I am using FormidablePro but that does not include a dataset just entries. Also, I’m developing on my laptop and wondered why a copy of the pdf is to going to http://www.idealchoiceinsurance.com instead of localhost?

The PDF is going to our server to use the PDFtk cross-platform tool. If you install PDFtk on your server, the PDF will stay on your server. NOTE: Signatures and images are not supported when using the local PDFtk option found on the plugin’s SETTINGS tab.

The [empty] text comes from the “Use as Dataset Name?” radio button field found on the field map layout, it means the selected field is blank. Change the selected field to a field that uniquely defines the entry (dataset) and save the field map layout.
To use the local version of PDFtk, you must select the “Enable local PDFTK” option on the plugin’s SETTINGS tab, there is no need to edit the fpropdf.php file.

If “actual screens don’t match” and “there is nothing showing below on my screen.”, then you have not uploaded a Adobe Acrobat compatible PDF Form (with existing acrobat form fields).
We are willing to login and take a look at your installation or you can upload the PDF and a screenshot of the field map layout with the Form and Field Map Layout selected at the top to this form. Then, we will be able to provide specific instructions so you may successfully use the plugin.
Alternativly, you may download the beta version of the replacement plugin called E2Pdf, which is much more intuitive, however, instruction videos will not be released for a couple of days.
