We have not experienced any issues as you have indicated. Please confirm the following;
1. Regular Formidable Forms are able to send emails?
2. Add an Email Notification action in the Formidable Form setting of the Form referenced in the plugin’s Field Map Layout?
3. From the plugin screen, you are selecting the saved Layout from the “Field Map to use:” dropdown
4. Change “Attach file to Email notifications” to YES?
5. Click SAVE at the bottom of the Field Map Designer?
