I have used you form for the last year and I have just renewed my licence. However my form is no longer functioning properly. It is not sending the user the PDF in the proper format and also to the sender. What information do you need from me in order for you to assist? The Plugin settings are telling me that the site is not activated and when I try to do this I receive a message to tell me that I have reached the limit of the websites that I can activate. Everything historical is in the plugin and the backups. I just cannot see the custom layout that you created for me on the form from last year.

Your license is active, we see no issues on our side. Complete this form if we can login or if not possible, then use the form to upload screenshots of the following so we can begin to troubleshoot the issue…
