    this plugin simply does not work. I have watched you short demo video and followed the instructions but still end up with a mostly blank form. the only field that populates is one signature field 21 fields.

    I thought I might add that I have PDF form with 18 fields. They consist of 2 signature fields that share the same value , 11 Initials fields that share the same value, 2 date fields that share the same value, 2 agent ID fields that share the same value, 2 name fields that share the same value.

    I created a formidable form with 4 fields: Name field, date field (current date default value), an Initials signature field, and signature field.

    I mapped the 5 formidable field to the 18 pdf fields. Maybe there’s an issue with using the same field more than once.

    The plugin works very well, everyday, for nearly 2000. The same field may be used multiple times without any issue.

    form was submitted.. I forgot include my activation key but you can see that I’m a paid member when you login. thanks for the help..

    Also I should have said the plugin “simply doesn’t work for me” no pun intended.

    After logging in we are unable to find any issues with the merge or field map layout.

    There are some PDF fields named “initialsbox” that are not actually on the PDF. Please check your PDF field names and map them to the correct Formidable form field names. Or, better yet, name the PDF fields the same as the Formidable Fields so the plugin will automatically map them when creating a new field map layout.

    When I created the document I named all of the field. “Initialsbox” is a field that I named that needs initials and is a name on the form.

    The PDF’s are still coming up empty except for one field. Please check again and try yourself.

    We see our plugin is finding the initialsbox fields, but there are actually NO PDF fields named “initialsbox” when you open your PDF (attached). In Acrobat and PDFEscape.com were are unable to view the textbox fields, however the fields named text14, text15, ect… do show up and are able to be mapped, see here. It appears your PDF is corrupt as the calculated full name field also has issues. It works on this test PDF merge, but not your PDF.

    We’ve mapped text15-18 to show you they work, however, we recommend deleting ALL fields from the PDF and recreating the PDF fields. We would only charge you $15 to create new PDF and mapping for you, complete this form if you’d like to hire us.
