I am still having the same problem. I created my PDF in adobe indesign. I then opened it in adobe acrobat and created the input fields and saved that pdf again. I can map the fields to the fields i created in formidable which is something i cannot do with your pdf, not sure why that is. The problem I am having is the attached pdf in emails sent after the form is submitted. Even with your test pdf the attached document is an error.txt file with the following message.

There was an error generating the PDF file.
This activation code is not valid.

I need the information submitted on the form to be attached in the email to the admin when the submission is made. I was under the assumption that this plugin could do that. Does it matter if i save the pdf as a high quality pdf vs. a reduced file size pdf or anything like that?
