Remove the quotes around [id] and try the link again. If the issues are not solved, try changing the “Flatten” setting to just YES, not the “and transform text to images” option.

Also, try one more thing if the above does not correct the issues, clear the “PDF File Name in Emails” box in the field map layout, the Layout Name will be used as default.January 5, 2016 at 10:03 am #5359
There seems to be two field map layouts set to email with the form submission.
Deselect “Attach PDF to email notifications” on the one layout that you are not using, and try exporting a pdf from the field map layout from the layout you are using to be sure it is merging correctly from inside WP.January 5, 2016 at 10:44 am #5360
