check boxes and radio buttons do not work

Hi have tried everything to get check boxes to work and no success. Can you please supply a simple, step by step guide to making this work with screen grabs. Last attempt had a form with just one text field, and one radio and one check box. Check box on my PDF was called ‘check1’ and I used the same name for the check box field in the form and for the check box option. Still no luck. Is it how the PDF is created that can cause the issue? Need a fix for this urgently.

You MUST change the PDF Checkbox field’s “EXPORT VALUE” to match the Formidable Checkbox field’s option value… (Use Copy/Paste) Currently, all your PDF field export values are set to “YES”. See the screenshot that was emailed to you…
