Here’s the syntax I used (102 is a name and 103 and 104 are date fields):
[102] | [103:mm/dd/yy] | [104:mm/dd/yy]

I added the format syntax to the current field map and it still outputs:
Benjamin | [103:mm/dd/yy] | [104:mm/dd/yy]

Do I need to delete the original field map and redo?
