    I have the form mapped and ready to go, but when I try to “preview” the PDF, it takes me too a blank page that says “Formidable PRO2PDF” in large black letters.
    When I tried to fill out a test form, the email never came through, but is logged showing that it was sent.
    The form I am creating is a long form, a little over 200 fields, should that make a difference? I did create a small test form with 4 fields, and it worked just fine.
    Please help me!
    I can send screen shots, if you tell me where to send them, do not see a place to upload them here.

    You may have a PDF form field issue. Be advised, the plugin is not compatible with AdobeLive Cycle forms or Adobe Signature Fields (use text fields instead).

    1. Try creating a new field map layout with the same large PDF form but only map the first few fields, if it works, duplicate the large field map layout and begin deleting field mapping and test the preview until the merge works as a process of elimination to find the field mapping that is causing the issue.

    I have been trying to work on this. I can now see the PDF when I preview it, but for some reason, I can not get the “checkbox” and “radio” box to show on the PDF as to which one should be checked. I have tried changing all aspects of the field, meaning, I have tried by changing the PDF field values, field names, and trying different boxes such as “checkbox” , “radio box”, and even as “text” box.
    I have even tried changing the “format” when mapping to the formidable form.
    Is there a way I can send you the PDF and have you see what I have done wrong? Or helping me get the mapping done?
    I have another form that I need to do as well, but if you can help me with what I am doing wrong on this one, then I should be able to do the other.

    Sure, we are willing to login to your installation or, you can export the template and attach it to this form.

    Formidable PRO2PDF → Templates → Export field map → SELECT Field Map → CLICK Export field map

    I have sent the form to you, along with login credentials for the site.
    I attached the template and the last PDF form that I have been playing around with.

    We have made the following changes to the field names and values of the “Social History:” section of the PDF Form, and updated your field map layout with the new PDF form.
    – ALL Radio Buttons and Checkboxes work in the Social History: section
    . Radio Buttons, changed name of each PDF field to be the same, and changed the values to match the respective Formidable Form values.
    . Checkboxes, changed the name of each PDF field to be unique, and changed the values to match the respective Formidable Form values.
    . Mapped each Formidable Radio Button to the (1) PDF field name.
    . Mapped the Formidable Checkbox to the EACH of the PDF field names.

    I think I just kept getting my self confused on how to label the filed name and field value.
